Beginning Dec. 22, medical examiners were required to use a new Medical Examina- tion Report Form and Med- ical Examiner's Certificate when giving DOT physicals. FMCSA now is giving exam- FMCSA grants grace period for medical forms iners until April 20 to fully transition to the new forms "to ensure that med- ical examiners have sufficient time to become familiar with the new forms and to program electronic medical records systems." FMCSA said the new forms were not available to medical examiners un- til Dec. 14 due to technical difficulties. – Matt Cole MOUNIR R. KHOURI was debarred Nov. 23 for three years by the Federal Motor Carrier Safety Administration for his conviction related to a scheme to defraud transpor- tation companies employing truck drivers subject to random drug testing. Khouri did business as Mobile Testing Services, a third-party admin- istrator. In 2009, it was found that he falsified documents for untested urine samples, misrepresented that the sam- ples tested negative and billed for services not provided. THE OREGON DEPARTMENT OF TRANSPORTATION requested an exemption from the Federal Motor Carrier Safety Administration's re- quirement that a commercial learner's permit be valid for no more than 180 days after it is issued, after which the issuing state has the option to renew the permit for an additional 180 days. ODOT is requesting to extend the 180-day timeline to one year, saying the requirement to re- new the permit at six months seems unnecessary.
