Compile a digest overview of the event/campaign including representative examples of press releases, press clips, photos, a summary of the objective, etc. Please also include at least one jpeg image from entry on CD, representative of the campaign theme, for publication. For Press Kit and Press Release, please submit the actual piece and a CD with jpeg of entry. 45. PR Single Event Any one-time public relations, community affair or media event. 46. PR Campaign Any two or more events, for the same brand, with the same theme and objective. for these categories as described below. 49. Web Site Include the site's URL (address) on the entry form and submit jpeg of entries on a CD. 50. Web Advertisement An advertisement created for the Internet. Please include the web address (URL) where ad can be found on the entry form, and submit a CD with jpeg of entries. Ads must have run between April 2011 and May 2012. If ads ran during that time and are no longer on display, please include ads on CD with screen shot of where they appeared (also include dates they ran). 2013 Awards Entry Form The information requested on this form will be used to list all winning entries in Beverage Dynamics magazine and for printing award certificates. Beverage Dynamics reserves the right to re-categorize entries if they are submitted in an incorrect category. Entrants are responsible for providing accurate information, which will be published exactly as it appears on the form. Fill in only those items that apply. Two copies of the entry form are required for each entry. (Photocopy as many copies as you need.) Attach one copy to the entry and the other to the entry fee check ($175 per entry).
