Notice of Your Information is Safe Protecting your privacy is very important to the National Catholic Society of Foresters. The Society maintains the highest level of confidentiality concerning your certificate information with the NOTICE OF PRIVACY POLICY (at right). Our Members' Service Representatives have been trained to protect such information and are required to comply with our established policies and procedures. We will only give information on a certificate to the insured (or the owner, if different than insured). Please review the certificate under the "Control Section" or any endowments to see who owns the certificate. CHECK YOUR DESIGNATED BENEFICIARY We recommend that everyone check their beneficiary designation regularly. • Make sure the beneficiary named on your certificate is still living. • Make sure the beneficiary named is the person you wish to receive the proceeds on your certificate. If you need to verify the beneficiary designation on your certificate call the Home Office at 1-800- 344-6273. You have provided the Society with certain nonpublic personal information, such as your name, age, residence, marital status, and social security number. At the time you applied for life insurance or an annuity, you also provided us with employment and medical information and authorized us to obtain further information concerning your health history, avocations, and other personal characteristics. Based on this authorization, only the minimum amount of information necessary to underwrite your application has been collected. The Society maintains the highest level of confidentiality concerning your nonpublic personal information. Employees of the Home Office have been trained to protect such information, and are required to comply with our established policies and procedures. There are no affiliated financial institutions or third party non-affiliates of the Society which have access to your nonpublic personal information, except pursuant to your written authorization. We do not sell lists of names and addresses of our members to any vendor for goods or services. Our privacy policy also extends to former members who no longer have coverage with the Society. We do not disclose any nonpublic personal information about our members or former members to anyone except as required by law. Keeping your information accurate and up-to-date is very important to us. You may request and correct nonpublic personal information about you that we collect except for information relating to a claim or a criminal or civil proceeding. Federal law established new privacy standards and requires us to provide this privacy policy to you once each year. 31
