Below are some of our most frequently asked questions. Why can't you return the certifi ed death certifi cate? We are required to keep the certifi ed death certifi cate as part of the claim for auditing purposes. Can I call to report a death if I am not the benefi ciary? Yes, you can call to report a death claim if you are not the named benefi ciary. Please call the Home Offi ce and ask for Customer Care. I pay the premium; therefore I should be able to get information. In some cases, the person who pays the premium is not the owner of the certifi cate. Only the owner has the right to make changes or get information. I have accumulated dividends, can I request a portion of the dividends by just calling the Home Offi ce? You have provided the Society with certain nonpublic personal information such as your name, age, residence, marital status, and social security number. At the time you applied for life insurance or an annuity, you also provided us with employment and medical information and authorized us to obtain further information concerning your health history, avocations, and other personal characteristics. Based on this authorization, only the minimum amount of information necessary to underwrite your application has been collected. The Society maintains the highest level of confidentiality concerning your nonpublic personal information. Employees of the Home Office have been trained to protect such information, and are required to comply with our established policies and procedures. There are no affiliated financial institutions or third party non-affiliates of the Society which have access to your nonpublic personal information, except pursuant to your written authorization. We do not sell lists of names and addresses of our members to any vendor for goods or services. Our privacy policy also extends to former members who no longer have coverage with the Society. We do not disclose any nonpublic personal information about our members or former members to anyone except as required by law. Keeping your information accurate and up-to-date is very important to us. You may request and correct nonpublic personal information about you that we collect except for information relating to a claim or a criminal or civil proceeding. Federal law established new privacy standards and requires us to provide this privacy policy to you once each year.
